Definition: Operator error, more commonly known as human error, is when the operator of the equipment is responsible for an unintended consequence. It is when the operator makes a bad decision, ignores directions, becomes distracted or does something out of character while operating a machine or system and causes a mishap or accident.

A car accident caused by the driver is caused by operator error.

A more thorough definition for operator error is using it as a concept that explains an accident that occurs when operating a device or system where the error the error that caused it occurred due to the uninfluenced actions of the human operator.
